Overview

What Feedback Form (Common Ninja) is

Feedback Form is Common Ninja's survey-style form widget, pre-tuned for ratings and comments and embedded on a Squarespace page with one line of code. The old copy here described buttons, tabs and accordions, a different product. It is their Form Builder engine with defaults for satisfaction surveys.

What it does

The vendor page lists star and scale ratings, multiple choice and checkboxes, dropdowns for categories, long-form text for open comments, and file or image upload for screenshots. Fields can be grouped under section headers and reordered by drag and drop. Notification emails go to whoever owns each feedback flow, honeypot fields and optional CAPTCHA keep junk out, and every response is saved to the Common Ninja dashboard and exportable to CSV.

Who it suits

Product feedback, customer satisfaction surveys, post-purchase and event feedback, staff feedback and feature requests on a Squarespace site, without paying for a separate survey tool.

One honest caveat

Squarespace's Form Block can collect a comment box for free, but has no rating scales or file uploads, which is the real reason to use this. In return your responses live in Common Ninja's dashboard on a third party script, and there is no reporting beyond the list and CSV export, so analysis happens in a spreadsheet.

Setup and pricing

Build the form in Common Ninja's editor and paste the embed code into a Squarespace code block (Core, formerly Business, plan or higher). Common Ninja is recurring software: the free plan covers one feedback form with no time limit, and paid tiers add forms, submission volume, premium skins and the advanced features noted above.

FAQs

FAQs

What does the Common Ninja Feedback Form do on a Squarespace site?
It adds a hosted feedback or survey form with star and scale ratings, multiple choice, dropdowns, open text and file upload, with responses saved to the Common Ninja dashboard and exportable to CSV.
How is it different from the Squarespace Form Block?
The native block can take a comment, but has no rating scales, no file uploads and no shared dashboard. The widget adds those on a third party script.
How is it different from Common Ninja's Form Builder?
Same engine, different defaults. Feedback Form starts with rating scales, multiple choice and open comments already in place so you are not building a survey from scratch.
Can I get reports or charts from the responses?
Not inside the widget. Responses appear as a list in the dashboard and export to CSV, so charts and averages happen in a spreadsheet.
How do I add it to Squarespace?
Build the form in Common Ninja's editor, copy the one-line embed snippet and paste it into a code block on the page. Code blocks need a Core (formerly Business) plan or higher. Later edits in the editor update the live embed automatically.
Is it free?
Common Ninja's free plan covers one feedback form with no time limit and no credit card. Paid tiers add more forms, higher monthly submission limits, premium skins and payment collection, advanced field types, webhook integrations and conditional logic. Check the current tier table on the vendor site before relying on a specific feature.
