Online Form Builder

Online Form Builder is Elfsight's Form Builder widget, a general-purpose form tool for Squarespace that covers contact, feedback, survey, booking, quote and registration forms. It is an embeddable widget hosted on Elfsight's platform rather than native Squarespace code. In Elfsight's editor you build from 22 field types, including file upload, signature capture, ratings and date pickers, add conditional logic so fields appear based on earlier answers, and split long forms into steps. Layouts are Default, Boxed or Floating (an expandable icon that can sit on every page). Submissions go to your Elfsight dashboard with email notifications to up to three recipients plus CC and BCC, an optional custom SMTP so emails come from your domain, and one-click integrations with Google Sheets, Mailchimp and Zapier. reCAPTCHA, saved progress and an AI generator that drafts a form from a text prompt are included.

Who it suits

Businesses whose forms have outgrown the native Squarespace Form Block: quote requests with conditional questions, applications with file uploads, surveys with ratings, or anything that needs a signature. A free plan is available to start with.

One honest caveat

For a simple contact form the native Form Block is free, stores submissions in Squarespace and needs no third-party script. Choosing Elfsight means your form data lives on Elfsight's servers, which has data protection implications you should think through. Free-tier widgets carry Elfsight branding and monthly view limits, and the form stops working if your account lapses.

Installation

Build the form in Elfsight's editor, copy the embed code and paste it into a Squarespace code block, or into footer code injection for the Floating layout. Later edits in the editor update automatically.
